Pursuant to Senate Concurrent Resolution 107, Senate Draft 1, House Draft 1, the 2015 Legislative Family Caregivers Working Group is convening to examine and assess issues related to family caregivers.

The purpose of this informational briefing is to brief the Legislative Family Caregivers Working Group on the current discharge processes in Hawaii’s hospitals, describe the practices of designating and involving family caregivers in the discharge process for patients at Hawaii’s hospitals, and receive updates about initiatives to improve discharge planning and transition care for patients.  Invitees to this Informational Briefing include:

· Designated representatives of Healthcare Association of Hawaii’s member hospitals;

· Jayme Puu, Executive Director, Provider Relations, Jennifer Diesman, Government Relations, and/or designated representatives of Hawaii Medical Service Association; and

· Lois Nash, Director of Pharmacy, Hawaii Pacific Health and Pali Momi Medical Center.

No public testimony will be accepted.
