                    SENATE  CONCURRENT
                        RESOLUTION

  REQUESTING THE STATE HEALTH PLANNING AND DEVELOPMENT AGENCY TO
    REQUIRE DETAILED JUSTIFICATION FROM THE HAWAII HEALTH
    SYSTEMS CORPORATION IN ANY CERTIFICATE OF NEED PROPOSAL TO
    REDUCE LONG-TERM CARE BEDS. 


 1        WHEREAS, according to statistics of the State Health
 2   Planning and Development Agency (SHPDA), in 1998 there were
 3   4063 long-term care beds statewide, with an occupancy rate of
 4   90.45%; and
 5   
 6        WHEREAS, the facilities of the Hawaii Health Systems
 7   Corporation (HHSC) are significant providers of long-term care
 8   beds as shown in the following statistics reported by SHPDA for
 9   1998

26        WHEREAS, these beds are primarily utilized by those
27   persons aged sixty and over; and
28   
29        WHEREAS, between 1990 and 2010 this age group is expected
30   to grow by seventy-two per cent as compared to a total
31   population growth of only twenty-nine per cent; and
32   
33        WHEREAS, with this increasing elderly population the
34   demand for long term care beds will not decrease; and

 2        WHEREAS, there have been reports that the Hawaii Health
 3   Systems Corporation is considering the reduction or elimination
 4   of long-term care beds in a plan to move patients from
 5   institutionalization to home- and community-based care; and
 6   
 7        WHEREAS, the reduction or elimination of long-term beds
 8   requires prior certificate of need approval from SHPDA; now,
 9   therefore,

11        BE IT RESOLVED by the Senate of the Twentieth Legislature
12   of the State of Hawaii, Regular Session of 2000, the House of
13   Representatives concurring, that for purposes of reviewing any
14   certificate of need application from the HHSC to reduce long-
15   term care beds, the SHPDA is requested to require the HHSC to
16   address the following in their application:
17   
18        (1)  An assessment of the impact of the proposed
19             elimination of the facility's long-term care beds;
20   
21        (2)  An assessment of the long-term financial implications
22             of transferring the patients to other hospitals or
23             existing facilities;
24   
25        (3)  An assessment of the ability of the corporation to
26             place their existing patients in comparable private
27             facilities;
28   
29        (4)  An assessment of the prospects of developing
30             alternative delivery systems that maximize the use of
31             elder home care health services and other support
32             services designed to extend physical and financial
33             independence in existing residential settings; and
34   
35        (5)  An assessment of national standards or literature for
36             criteria to determine which long-term patients can be
37             served in a home or community based setting, and
38             which require the services of an institutional
39             setting, and an application of these criteria to the
40             facility's patients to determine the most appropriate
41             setting for their care;

 2   
 3        BE IT FURTHER RESOLVED that the HHSC collaborate with the
 4   State Department of Human Services regarding home and community
 5   based services, and the Executive Office on Aging (EOA), in
 6   preparing the application for a certificate of need; and
 7   
 8        BE IT FURTHER RESOLVED that the EOA and the SHPDA partner
 9   to provide the legislature with market data regarding elderly
10   demographic and long term care utilization statistics, and
11   report to the legislature twenty days before the convening of
12   the Regular Session of 2001; and
13   
14        BE IT FURTHER RESOLVED that certified copies of this
15   Concurrent Resolution be transmitted to the Administrator of
16   the State Health Planning and Development Agency, the Director
17   of the Executive Office on Aging, the Director of the State
18   Department of Human Services, and the President and CEO of the
19   Hawaii Health Systems Corporation.
