                     HOUSE CONCURRENT
                        RESOLUTION
REQUESTING THE STATE HEALTH PLANNING AND DEVELOPMENT AGENCY TO
   REQUIRE DETAILED JUSTIFICATION FROM THE HAWAII HEALTH
   SYSTEMS CORPORATION IN ANY CERTIFICATE OF NEED PROPOSAL TO
   REDUCE LONG-TERM CARE BEDS.



 1       WHEREAS, according to statistics of the State Health
 2   Planning and Development Agency (SHPDA), in 1998 there were
 3   4,063 long-term care beds statewide, with an occupancy rate of
 4   90.45 percent; and
 5   
 6       WHEREAS, the facilities of the Hawaii Health Systems
 7   Corporation (Corporation) are significant providers of long-
 8   term care beds as shown in the following statistics reported by
 9   SHPDA for 1998:

27       WHEREAS, these beds are primarily used by persons aged
28   sixty and over; and
29   
30       WHEREAS, it is expected that between 1990 and 2010, this
31   age group will grow by 72 percent as compared to a total
32   population growth of only 29 percent; and
33   
34       WHEREAS, with this increasing elderly population, the
35   demand for long-term care beds will either remain status quo or
36   increase; and

 1       WHEREAS, concerns have been raised with regards to any
 2   attempt by the Corporation to reduce or eliminate its long-term
 3   care beds by moving patients from institutionalism to home- and
 4   community-based care; and
 5   
 6       WHEREAS, the reduction or elimination of long-term care
 7   beds requires prior certificate of need approval from SHPDA;
 8   now, therefore,
 9   
10       BE IT RESOLVED by the House of Representatives of the
11   Twentieth Legislature of the State of Hawaii, Regular Session
12   of 2000, the Senate concurring, that SHPDA is requested to
13   require detailed justification from the Corporation in any
14   certificate of need proposal to reduce long-term care beds; and
15   
16       BE IT FURTHER RESOLVED that before reviewing any
17   certificate of need application from the Corporation to reduce
18   long-term care beds, SHPDA shall require that the Corporation
19   satisfactorily address the following issues as part of the
20   application:
21   
22       (1)  An assessment of the impact of the proposed
23            elimination of the facility's long-term care beds;
24   
25       (2)  An assessment of the long-term financial implications
26            of transferring the patients to other hospitals or
27            existing facilities;
28   
29       (3)  An assessment of the ability of the Corporation to
30            place their existing patients in comparable private
31            facilities; and
32   
33       (4)  An assessment of the prospects of developing
34            alternative delivery systems that maximize the use of
35            elder home care health services and other support
36            services designed to extend physical and financial
37            independence in existing residential settings;
38   
39   and
40   
41       BE IT FURTHER RESOLVED that certified copies of this
42   Concurrent Resolution be transmitted to the Administrator of
43   SHPDA and the President and Chief Executive Officer of the
44   Corporation.
